The following documentation is required to be submitted if it is relevant to the situation for which the grant is being sought: 1) a copy of the AWS Order of Suspension, AWS Order of Revocation or AWS Facility Compliance report citing the specific violation(s) for which the county is applying for 2) the date and description of the incident and damage incurred by the unforeseen catastrophic disaster such as a hurricane, tornado, fire, flood, or other natural or man-made disaster at the animal shelter; 3) an explanation of how the applicant will use the grant funds to resolve or mitigate the cited violation or violations of the Animal Welfare Act and its rules and how the applicant will ensure future compliance with the AWA and its associated rules; and 4) an itemized listing of the costs for which funding is sought.
