Grant Guidelines | Austin Bailey Health and Wellness Foundation The Austin-Bailey Health and Wellness Foundation is committed to supporting programs that promote the physical and mental well being of the citizens of our community. Those seeking Foundation funding must serve all or part of the four-county area of Holmes, Stark, Tuscarawas and Wayne in Ohio.
Austin-Bailey Health and Wellness Foundation does not generally grant funds for the following: Organizations not classified as tax exempt by the IRS Annual appeals or membership drives Non-specified capital campaigns or fundraising events Proposals which have not been prepared according to the guidelines provided will be considered ineligible.
We ask that you call the Foundation office at (330) 580-2380 to request a grant application and guidelines. Following our discussion, you will receive specific instructions by mail or email to assist you in this process. Since incomplete proposals cannot be considered for funding, it is important that you follow the procedures outlined in our correspondence to you.
The trustees of the Austin-Bailey Health and Wellness Foundation want to give your proposal the consideration that it deserves. Your grant application is the most important stage in our evaluation of your proposal. For this reason, you should address each question asked specifically and completely.
The first step in the application process is to complete a “Grant Request Cover Sheet,” which is available from the foundation office. This form will assist our board in determining if your request will be a match with our mission in the community.
You will be asked to provide the following: Description of your organization Objective(s) of the project/program IRS Tax determination letter – 501(c)(3) indicating tax exempt status Prior year’s actual financial statement List of your board of directors This information should give our review committee sufficient data to make a decision about the grant request. If not, we will follow up with a letter asking for clarification.
Grants are awarded twice a year. Application deadlines are May 26, 2026 and November 17, 2026. Applications not received by the close of business at 4:30 p.
m. on the deadline date will not be considered. The Foundation’s board of trustees meets in March and September to review grant applications.
Applications received by the May deadline will be approved in mid-September. Applications received by the November deadline will be approved the following March. Applicants will be notified shortly thereafter of the board’s decision.
