</li></ul> The CCPD Grants Program seeks to build a cohesive, comprehensive approach to reduce chronic diseases in Colorado by focusing on prevention, early detection, and treatment for cancer, cardiovascular disease and pulmonary disease.
The CCPD Review Committee adopted a strategic framework that continues the direction of the Fiscal Year (FY) 2016-18 grant funding cycle and aligns with the Chronic Disease State Plan specifically focusing on three chronic disease domains: <ul><li>Policy and Environmental Approaches: population-based strategies that transform communities to support health and make healthy behaviors easier and more convenient for people;</li><li>Community-clinical Linkages: strategies that ensure linkages between clinical or health care settings and effective, evidence-based community resources for people with chronic disease and those at risk for chronic disease.

png" border="0" style="cursor: pointer; position: relative; top: 3px;" /> This RFA seeks to fund applications addressing one of four potential focus areas Policy and Environmental Change, Health Systems Transformation, Community-clinical Linkages, or Health Systems combined with Community-clinical Linkages.
Applicants are expected to select one focus area and to develop an application describing how they will achieve and advance the goals of that focus area over the next three (3) to five (5) years.
Having selected a focus area, applicants must then select One Approach from the following two options: Approach A: to address specific strategies within that focus area; OR Approach B: to propose an evidence-informed innovation that directly links to and supports that focus area and advances the strategic goals of the CCPD Grants Program.
This document provides an overview of each strategy as well as suggested objectives, measures, intervention strategies, activities, and reporting metrics. Funds from this grant may not be used for: Activities or projects that will have been completed before funding becomes available (no retroactive funding). Proposals may not supplant existing funding.
Capital projects, maintenance or equipment as direct costs. Equipment is defined as a purchase greater than or equal to $5,000 for an article of nonexpendable, tangible personal property having a useful life of more than one year.
If selected as a grantee, grantees will be required to certify that no direct or indirect affiliation, contractual relationships, or receipt of funds or resources exist with tobacco companies or owners, affiliates, subsidiaries, holding companies, or companies involved in any way in the production, distribution, promotion, sales or use of tobacco products.
