- 6/30/25** * The new form no longer requires the provider’s signature. * There is a new check box! If checked, it allows the DHHS Eligibility Specialist to share additional (more than what is listed on the DHS-198-Child Development and Care Provider notice) case information with the provider.
The most current version of the MDHHS 4025 was revised on 06-2025 and previous versions will not be accepted. It must be completed per the instructions on the form. If the previous MDHHS 4025 Provider Verification is received by MDHHS, the provider will not be assigned, and the Eligibility Specialist will send the parent a newMDHHS 4025 Provider Verificationfor completion.
**News Regarding I-Billing - 6/18/25** I-Billing is only accessible through anMiLogin for Citizens account. Providers who already have an MiLogin account should add I-Billing to their existing account. If you do not already have a MiLogin account, you will need to create one to continue to access I-Billing.

Review theCCDF State Plan for FY25-27 * Grant Information (Updated on 12/9/2024) **One-time Operational Grant** MILEAP is committed to ensuring all children have access to high quality early learning opportunities and are happy to announce we are preparing to distribute $24M amongst 3,956 licensed providers in 82 counties. The valuable care provided to eligible CDC scholarship children is being recognized.
Eligible licensed providers who provided care between February 2024 and September 2024 will receive a payment that was determined according to legislative requirements based on age of child, full-time and part-time care,and quality level. No eligible provider will receive less than $485. Amounts determined are final and will not be changed or modified by the Department.
Separate payments will be issued via SIGMA/EFT for this one-time operational grant. ### **Frequently Asked Questions** **What are one time operation funds and who is eligible to receive? ** A $24M legislative appropriation to issue a one-time payment to licensed child care providers who received a CDC scholarship payment between February 2024 and September 2024.
**Do I have to fill out an application? ** No. The Department will issue a one-time payment using administrative scholarship billing data from Bridges. **How did you determine how much money providers receive?
** The Department utilized the legislative boilerplate language: _(1) From the funds appropriated in part 1 for child care program stabilization grants, an amount not to exceed $24,000,000. 00 shall be used for additional supports to licensed child care providers in this state who serve children in the child development and care program.
The funding must be distributed to eligible providers in a form and manner determined by the_ _department, but must take into consideration all of the following:_ _(a) The age of the children served by the provider. _ _(b) The quality rating of the provider. _ _(c) The length of time the children are in care with the provider.
_ _(2) The department shall provide technical assistance to_ _providers on the recommended uses of the funding described in_ _subsection (1). Funding must be distributed to providers no later_ _than January 1, 2025.
_ **Payments calculations:** **Age of Child****Amount Per Child** Quality Level of the Provider is Reflecting on Quality (level 2) or Higher$125/child if the provider was participating in GSQ at Reflecting on Quality or higher between February 2024 and September 2024 _Average Length of Time the Child is in care (each child is either considered part-time or full-time)_$100/child if provider averaged payment for 30 hours or less between February 2024 and September 2024 (**half-day**) $175/child if provider averaged payment for more than 30 hours between February 2024 and September 2024 (**full-day**) **Example - Provider is caring for 12 children:** 1 infant/toddler ($400) + 2 preschoolers ($250) + 9 school agers ($900) = $1,550 Provider is participating in GSQ at Reflecting on Quality or higher (12 x $125) = $1,500 Provider paid for 3 children half day (3 x $100) = $300 Provider paid for 9 children full day (9 x $175) = $1,575 **What can I use the funds for?
** The payments are being made using general fund dollars. These funds are intended to help support staffing costs, offering staff bonus payments, or to cover operating costs. **What am I not allowed to use the funds for?
** There are no restrictions on the use of these general funds. **Will I be required to submit a report? ** No reporting will be required.
**Are these funds taxable? ** The one-time operational grant is considered income and is taxable. Programs that receive more than $600 annually from the state will receive an IRS Tax Form 1099-NEC that indicates total funding received from the State of Michigan.
This funding is subject to the same tax rules as regular CDC scholarship funding. State tax rules apply. Regarding federal tax rules, please contact your tax preparer or the Internal Revenue Service for guidance.
In some cases, funds used to cover operating expenses may be exempt from taxation. We strongly urge you talk with your tax preparer to create a plan for how to estimate the tax implications for planning purposes. **Can these funds be used to help cover the costs of consulting with a tax preparer?
** **Can these funds be used to pay for employment taxes and property tax? ** **Can I use these funds to pay state or federal taxes associated with receiving the grant funds? ** **What if I do not wish to keep the funds?
** If you receive these funds and do not wish to accept the one time operational grant you must contact the Department to return the funds immediately. Please email MiLEAP-CDC-Admin MiLEAP-CDC-Admin@michigan. gov with the following information: * Provider license number * Reason for returning the funds Someone from the CDC program office will follow up with instructions for returning the funds.
Failure to notify the Department immediately may result in a 1099 be issued with the one-time grant payment included as funding received in 2024. If you believe there are errors on your 1099 you will need to contact Treasury to resolve the issue. ### Summer 2022 Child Care Stabilization Grants For the Summer Stabilization Grant we will release on August 9 and close on September 9.
Report will be due no later than July 1, 2023 (all unspent funds returned by July 15, 2023). Award letters to be issued September 30 with payments to providers the first week of October. Follow the Summer grant page link for webinar information and grant updates as they become available.
