Our Grants - The HealthCare Foundation of Cleveland County Our organization awards grants for innovative programs or initiatives that address targeted healthcare concerns in Cleveland County.
As part our mission, the Foundation may award Community Health Grants to qualified non-profit organizations in Cleveland County who seek support for innovative or needed programs or initiatives that address targeted healthcare concerns in Cleveland County and/or the social determinants of health and well-being. We value diversity, stewardship, integrity, compassion, respect, service, and leadership.
Grants will be announced/awarded during three annual cycles as follows: Deadlines for Submitting Completed Applications: Tax exempt organizations may submit one request per calendar year by completing a Grant Application Form and required documents on or before each grant cycle deadline date to be considered.
Community Health Grants are awarded at the sole discretion of the Board of Directors, based on the total amount of unrestricted funds available and allocated for community health grants each calendar year.
Areas typically NOT funded include, but are not limited to: Capital campaign requests Debt retirement/deficit funding or expenses incurred prior to grant award decisions Direct support of individuals (e.g. scholarships, hardship) Duplication of existing programs or services by new organizations Projects that are typically the funding responsibility of federal, state or local government Projects that promote political or religious views Political or lobbying activities Sponsorships or fundraisers To begin the grant application process, grant seekers should request a copy of the Foundation’s grant application or download it by clicking the link below.
The application contains brief prequalification questions to determine if the applicant’s organization is eligible to apply for a grant.
Qualified organizations will be required to provide: General information about the organization A narrative that summarizes the project, including the need(s) the organization aims to address, other partner organizations (if applicable), and how the organization plans to evaluate the project Results the organization hopes to accomplish through the project A detailed project budget and information about other funding sources Supporting documents as requested.
A final decision about the grant application is made by the board of directors. Applicants may receive full or partial funding or may be declined funding. Applicants will receive notification of the decision via a phone call, email or letter.
Within two weeks of approval, the applicant organization will receive an award agreement to be signed along with the reporting requirements. Upon receipt of the signed award agreement, a grant check will be mailed shortly thereafter. Organizations that receive a grant may be asked to submit quarterly and/or a final report, as specified in the award agreement.
This report allows the Foundation to measure the impact of the program as well as to confirm that the funds granted were used for the intended purpose.
