Fidelis Care at Home (MLTC) Over-the-Counter Benefits Mail Order Pharmacy Program Health Insurance Terms Explained Medicare and Dual Advantage Prescription Drug Information Medication Therapy Management Home Providers Provider Resources Maternal Health Grant Fidelis Care Maternal Health Grant Program Fidelis Care is awarding grants to healthcare providers and community-based organizations in New York State to help support maternal health and wellness programs.
This year's program is focused on funding innovative strategies in postpartum care and maternal mental health. Please ensure your application connects to these goals. If your office or organization runs a program that helps supports postpartum care and maternal mental health, particularly for high-need, low-income women and their families, then Fidelis Care encourages you to apply for the grant.
Grants up to $20,000 will be awarded. To be eligible, providers or community organizations must serve New York State. Applications will be accepted until 5 PM on July 8, 2025.
Applicants must answer all of the questions on the form to be considered. Please note: Some reporting is required. Awards will be announced in August.
