We redistribute and share power with the people we serve through advancing participation in democracy, amplifying community-driven movements, and strengthening small, community-based organizations and leaders. In Fall 2025, Health Forward released an open funding opportunity to support organization strengthening and capacity resilience for small nonprofits and their leaders.
This funding will support efforts to strengthen operational capacity by improving financial management, human resources, and technology, while also focusing on leadership development and staff well-being to build a more resilient and adaptable organization for long-term sustainability. To support this work, we will award $100,000 annually or $200,000 over 24 months. We are no longer accepting applications.
Funding decisions will be announced in January 2026. How healthy we are is strongly influenced by our society’s systems, policies, and stories. In Fall 2025, Health Forward released an open funding opportunity to support efforts to reshape harmful narratives that contribute to racial, economic, and health inequities.
To support this work, we will award a total of $1 million in funding, across multiple organizations, for a 12-month period. We are no longer accepting applications. Funding decisions will be announced later this year.
We build health and wealth by improving access to safe, healthy, and affordable housing and homeownership. We’re also working to ensure everyone has the access and knowledge required to use digital tools. In 2024, we awarded $1.
6 million to six nonprofit organizations working to support healthy and affordable homeownership opportunities. In 2025, we awarded $515,000 to 17 nonprofit organizations working to advance digital access, skills, and equity throughout its six-county service area. We support organizations that provide whole-person care across the community health system from prevention to treatment to recovery and advocacy.
