M. CT Eligible applicants are programs, sponsors, and potential sponsors of rural residency programs located in Minnesota.
Eligible programs all the following criteria: Train, or propose to train, medical residents in: A new rural residency training program, or A new rural residency training track within an existing residency program, or A community-based ambulatory care center that primarily serves the underserved and is proposing to add a rural residency training program track.
For the purposes of this grant program, a rural residency training program means a program that provides an initial year of training in an accredited residency program in a rural or urban area of Minnesota. For programs in general pediatrics, general surgery, and psychiatry, training after the first year must be based in communities outside the seven-county Twin Cities metropolitan area, with rotations in rural communities.

Rural Health Transformation Program: Telehealth Access Points Intent to Apply Deadline: N/A Application Deadline: Tuesday, June 30, 2026, 4:30 P. M. CT The applicant may be either a healthcare provider or a site hosting the TAP.
Eligible applicants include: Local Public Health departments Long-Term Care facilities Community-based Organizations and Community Centers All applicants must demonstrate that the host site has a suitable space, or can be modified to create a suitable space, to provide telehealth in keeping with best practices and legal requirements, such as protecting patient privacy.
