Through our annual grantmaking process, we provide funding to local nonprofit partners whose programs improve access, coordination, and quality of services across Monterey and San Benito Counties.

We fund initiatives that: Advance hospice and palliative care services Expand grief and bereavement support Offer family and caregiver assistance Enhance education, planning, and communication around end-of-life care Build capacity and professional development for local providers Our priorities emphasize collaboration, measurable outcomes, and services that would otherwise be limited or unavailable without philanthropic support.

We do not fund for-profit organizations, political or lobbying groups, or programs outside our service area. On this page, you’ll find information about eligibility, funding priorities, and how to apply, including current deadlines and submission guidelines. Our goal is to make the grant application process clear and accessible , so organizations with aligned missions can focus on the impact they bring to the communities we serve.

To learn more, please email Mary Gunn, Senior Director of Grants & Impact: [email protected] HOW TO APPLY: New applicants and eligibility Applications will only be accepted from non-profit agencies that meet the following criteria: Services must be focused on end of life: hospice, palliative care, bereavement/grief, end-of-life preparedness, and support services for families of patients with life-limiting illness.

Must be a 501(c)3 registered non-profit. Programs serve residents of Monterey and/or San Benito Counties. Process for Organizations Not Previously Funded Consideration will be given only to agencies that meet criteria specified in funding priorities listed above.

Submit a letter of intent, no more than two (2) pages in length, by March 15 . If approved, agency will be directed to submit a full proposal.

Please download the Hospice Giving Foundation Letter of Intent and include: Organization name and primary contact person Mission, brief overview, and organizational structure Specific amount of request The program/project in 1-2 paragraphs along to address priority issue Primary goals and approach to measuring effectiveness Target population and/or region of service SUBMISSION GUIDELINES AND FORMS: Approved applicants Grant applications must be submitted online.

The submission must be completed by 5 pm on the stated deadline date – May 1. ​ You will not need to register to access the application. You may print a copy of the application for reference, save your work, and print a copy of your submission.

We recommend drafting the text in a Word document before entering the information. Statistical questions are included in the application. Required documents must be uploaded to the application.

A list of these documents can be found in the Grant Application Instructions which can be downloaded below. Please DO NOT send duplicate copies, mail or hand-deliver hard copies, or include additional brochures, videos, or promotional materials. Late submissions will not be considered for funding.

Note; request to submit a proposal does not guarantee funding. May 1: Final Reports Due – New Online Format May 1: Due Date for Grant Requests for current grantees and new applicants who received approval to apply. August: Final recommendations to the HG Foundation Board of Directors.

August/September: Award letters and grants distributed. GRANT REPORT REQUIREMENTS: Hospice Giving Foundation requires final reports and statistical updates from all funded agencies. Interim reports are required only specific agencies who are notified of the requirement by Hospice Giving Foundation.

Reports allow an agency to provide details about accomplishments, progress on stated goals, and challenges faced during the grant term. Reports inform HG Foundation, as well as our stakeholders and donors, about: Impact of services in the community. Insights and learning from innovation in your work.

Areas of unmet need and emergency issues. December 1: Interim reports needed only from agencies informed of the requirement. Beginning in 2023-2024, report submissions will be done online via Jotform .
