Apply for a Community Health Grant Louisiana Healthcare Connections invests in strategic community initiatives to expand access to healthcare, increase preventive care, and improve health outcomes for our state's Medicaid population. 2026 Investment Grants will fund programs and/or services that impact maternal and child health Our 2026 Investment Grant application closed on May 29. Awards will be announced by June 29. Maternal and Child Health Strategic Priorities Programs and/or services that measurably improve maternal, infant, and/or child health outcomes and expand access to care are eligible for consideration.
Priority will be given to initiatives that address documented gaps in care, reduce health disparities, and demonstrate the potential for meaningful, sustainable impact.
Focus areas may include, but are not limited to: Enhancing the quality, coordination, and continuity of prenatal, postpartum, and infant care Expanding access to comprehensive prenatal, postpartum, and infant wellness services in rural and underserved communities Providing education and outreach that promotes preventive care and timely health screenings during pregnancy, with particular focus on HIV and syphilis Integrating maternal mental and behavioral health services into prenatal and postpartum care settings Addressing social drivers of health that influence maternal and infant well‑being, such as food insecurity, nutrition access, safe housing, and other non‑medical needs For one-time investments that build capacity or enhance service, in alignment with our strategic priorities Applications accepted on a rolling basis; awarded quarterly Apply for an Impact Grant Submission Deadline Anticipated Award Announcement February 1 March 16 May 1 June 15 August 1 September 14 November 1 December 14
