« Return to previous page Note: Rural Health grants are shifting to a two-year funding cycle where individual grants are awarded up to $20,000 over two years. Rural health grant recipients in 2026 will be ineligible to reapply in 2027, but can apply again in 2028. We aim to fund nonprofit, community-led solutions that address health equity in rural communities, including culturally-focused care.
We prioritize proposals from organizations that address: Domestic violence and sexual assault services and prevention Maternal and birth health Mental health and chemical health Primary care and sexual health for underserved communities Services that support the health of seniors in order to remain independent in their homes Funding is limited to organizations headquartered outside of the seven-county Twin Cities metropolitan area (Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington).
Priority is given to free clinics and federally qualified/community clinics over larger health systems. Funding requests for events, sponsorships, seminars, museums, camps, and general health and wellness activities including nutrition, exercise, etc. - are ineligible.
