This program targets the nation's neediest populations and geographic regions. Areas with an April 1, 2026 project period start date are eligible to apply. Funding is available for Community Health Centers (CHC), Migrant Health Centers (MHC), Health Care for the Homeless (HCH), and Public Housing Primary Care (PHPC).

Applicants must be a public or nonprofit private entity, such as a tribal, faith-based, or community-based organization that proposes to offer comprehensive primary care services to an entire service area.
