Completion of 40 Hour Training Reminder: Programs must notify DHS within 10 days of staff turnover or additions. Identify any current vacancies. Include the length of time the positions have been vacant.
If new positions have been established, but not filled, do you plan on keeping the position? Describe how your agency tracks personnel time for IDHS grant. Attach relevant time & effort policy, procedures, and related forms.
Do you plan to utilize consultants/sub-awardees/subrecipients to support grant requirements, such as, program deliverables, fiscal administration? If yes, list below and identify aspect of grant award supported. If yes, what previous experience do you have working with such?
Acknowledgement that you, as the provider agency, is responsible for the performance of any consultants/sub-awardees/subrecipients or other external partner and ensuring adequate monitoring? Describe mechanisms in place to ensure data accuracy and integrity.
