” The FY26 Cycle 1 awards include seven Health Partnership Grants and one Health Equity Fund Grant, with projects ranging from culturally tailored wellness programs to large-scale healthcare system support and legal advocacy to increase access to public benefits for underserved populations.
FY26 Cycle 1 Grantmaking Highlights Fresh Approach is developing and implementing the data, billing, and referral infrastructure needed to deliver CalAIM Community Support services to Medi-Cal patients in Santa Clara County. Korean American Community Services is strengthening its capacity to serve the Korean and broader AAPI community through culturally responsive food and wellness programming.
West Valley Community Services is improving self-sufficiency and better health outcomes through its Community Access to Resources and Education (CARE) program, combining food distribution, case management, and referrals for low-income families.
