Spay & Neuter Voucher — Animal Welfare Alliance of Southeast Missouri Please fill out the from below to receive the spay/neuter voucher. The Animal Welfare Alliance is currently accepting applications for $50. 00 spay/neuter vouchers from pet owners who need financial assistance.
A limited number of vouchers will be awarded on a monthly basis. The voucher will be valid for $50. 00 off the spaying or neutering of a cat or dog at any participating local veterinarian’s office.
It should be used within two months of issue; if a time extension is needed for special circumstances, your veterinarian’s office can contact AWA about the matter at (573) 415-9199 How many people in your household do you financially support? (Proof of income may be required at the time the voucher is awarded.) Which animal and service is requested?
Please list special considerations regarding your pet. Thank you for applying for this program! You will be notified by mail if we can assist you.
