Efforts within this portfolio include our partnerships with: Finge r Lakes Vaccination Program – The Finger Lakes Vaccination Program is a collaborative funding program designed to help partners execute tailored communications and innovative, vaccine distribution strategies ensuring equity, transparency and efficiently in COVID-19 immunization efforts. The program leveraged an unprecedented collaboration of university and health systems, public health departments, county administrations, and community nonprofit organizations, administering vaccine doses to over 33,000 individuals in 2021 and achieving large increases in vaccination rates in the most impacted zip codes and populations. Rochester/Monroe Anti-Poverty Initiative (RMAPI) – RMAPI is a community collaborative working to improve quality of life in Rochester by reducing poverty and increasing self-sufficiency. To do this, RMAPI is focused on workforce development and increasing accessibility and affordability of basic needs. ROC the Future – A Strive partnership engaging the community and community leaders to improve academic achievement starting with 3rd grade reading. The Health Foundation is a Roc the Future convener organization. Latino American Health Coalition – The Latino Health Coalition works to improve health equity for the Hispanic community in the Rochester-Finger Lakes region. The long-standing advisory group has addressed a variety of health concerns for Latinos, including youth risk behaviors, language barriers, economic stress and cultural competency. Members of the Latino Health Coalition include leaders from community nonprofits, hospital systems, insurers, business, government and other organizations. African American Health Coalition – The African American Health Coalition seeks to improve health equity for Black residents in the Rochester-Finger Lakes region. Through our Community Event Fund , we support events that promote health equity and advance initiatives in our nine-county region. This fund is open to Health Foundation grantees and other mission-aligned nonprofits. Annual dinners, luncheons, and galas Athletic fundraisers (golf tournaments, charity runs, etc.)
Health fairs, screenings, and vaccine clinics Educational community events Must have current 501(c)(3) nonprofit status Submit requests exclusively through our online portal (no phone or email requests) Request limit: up to $5,000 Ineligible Applicants & Activities: Government entities, political groups, or capital campaigns Events managed or funded through national headquarters. Local chapters must directly receive and manage funds.
Review Process & Timelines: Applications are reviewed quarterly, based on your event date (not the application date). Apply as early as possible—funding is limited and may run out before quarter-end. Deadlines: Apply at least 4 weeks before your event.
Late applications will be declined. Current-quarter events: Expect a decision within one month of applying. Future-quarter events: Notifications sent during the quarter of your event.
Please refrain from requesting status updates before the notification period. We fund one event per organization per calendar year . Submission does not guarantee funding.
