Improving quality of life for older adults and elders. 2023 represented the final year of a 3-year externally funded effort. The purpose of the project was to improve the quality of life for older adults, with a focus on american indian/alaska native and hispanic/latinx elders, in rural eastern washington by promoting healthy, active aging through community led care coordination and comprehensive medication management. While the program's efforts and accomplishments were extensive, the following represent a selection of program achievements: approximately 686 individuals were served through health coaching and advanced medication care coordination services between april 2020 and december 2023. Patient activation measure (pam) scores increased by 17% during this time. Additionally, among our advanced medication care coordination partners, we saw a 34.1% increase in cantril's ladder assessment scores across a program participant population of 170.

A contract with the washington department of commerce to achieve housing stability for persons relocated from targeted state of washington rights of way within the city of spokane. The pre-eminent task accomplished through june of 2023 was to close the largest homeless encampment in the state, "camp hope", which at one point had over 600 individuals occupying a single city block. Services included initially stabilizing the camp hope environment and identifying its residents. Services then transitioned to housing referrals, resource referrals, emergency assistance, outreach and move-in assistance among others. Based on data from february 2024, 529 individuals were enrolled in the program. Of those, 80 were identified as having obtained permanent housing, with another 74 in area shelters awaiting housing. A significant portion still need attention, with 116 living in places not meant for habitation and 251 whose circumstances are unknown. Outreach and placement efforts are ongoing.

An externally funded effort supporting the continued growth and stabilization of the "peers at the front door" collaborative, a perinatal peer support delivery model. Specifically for the 2023 year, supporting the integration of maternal and/or infant mental health and/or substance use consultation into the "peers at the front door" initiative and developing a shared approach and training to perinatal peer support. This involved building a cohort of collaborative organizations that use peers (those with lived experience) to meet the needs of vulnerable pregnant parents and their infants. For 2024 and beyond, the foundation stepped back from its intermediary role to allow direct funding to and oversight by the collaborative organizations.
