See Grant Application Information and Instructions webpage for more detail.
At the time of application, all applicants must submit the following documents: Uniform Application For State Grant Assistance (pdf ) . Applicants must use their SAM-registered legal name. Each application must be sent in a separate email.
Note: Page 1 of the application is pre-populated. Applicants only need to complete pages 2 and 3. The completed application must be emailed no later than October 8, 2020, 5:00 PM to the following email address DHS.
