Prior eligibility: Existing Adult Day Health (ADH) and Day Habilitation (DH) providers enrolled in MassHealth.
Adult Day Health and Day Habilitation Programs Community Inclusion Transportation Grant Program is a $2 million HCBS ARPA-funded grant from the Massachusetts Executive Office of Health and Human Services (EOHHS) that funds existing Adult Day Health (ADH) and Day Habilitation (DH) providers to expand transportation capacity for people receiving day services. Eligible providers enrolled in MassHealth can apply for up to $150,000 to purchase new vehicles, maintain or modify existing vehicles, or provide other accessible transportation options. The program's goal is to make ADH and DH programs and community outings more accessible for individuals receiving day services, supporting greater community inclusion. Only existing ADH and DH providers currently enrolled in MassHealth are eligible to apply. Contact EOHHS or MassHealth for application details and current status.
