Aging Grant Program is sponsored by The Harry and Jeanette Weinberg Foundation. The Foundation supports projects that enable older adults to live safely and comfortably in their homes, maintain independence, and stay active and engaged in their community.

Older adults often face unique social, physical, and economic challenges that can undermine their health and quality of life, as well as their ability to remain in their homes and connected with their community.

The Foundation supports capital projects for older adults throughout the United States and provides program and operating grants only in priority communities . Safe and Accessible Homes The Foundation supports projects that enable older adults to live in their home safely and comfortably and maintain their independence.

Projects must consider an individual’s goals and living situation and incorporate supportive services tailored to the needs of older adults. Examples of appropriate projects: Home modification and repair programs that also deliver a variety of other services, such as occupational therapy and case management. Programs that focus on health and overall well-being to prevent hospitalization and institutionalization.

The Foundation supports efforts to foster the best quality of life possible for older adults. Projects must connect older adults with a range of services that help them age independently and stay active and engaged in their community. Examples of appropriate projects: Affordable housing with services that allow residents to age independently.

Housing communities and services that foster a supportive environment for individuals who are lesbian, gay, bisexual, transgender, or queer. Coordinated, comprehensive medical and social services that allow individuals eligible for nursing homes to remain in their community. Shelters and comprehensive services for individuals fleeing physical, mental, economic, and sexual abuse.

Community Aging in Place, Advancing Better Living for Elders (CAPABLE) Program for All-Inclusive Care for the Elderly (PACE) Housing for LGBTQ Older Adults The Foundation has supported the expansion of this five-month program in nearly a dozen states. CAPABLE combines home repair services with a registered nurse and occupational therapy for older adults struggling with everyday activities.

Research shows CAPABLE has provided more than six times the return on investment, with roughly $3,000 in program costs per participant yielding more than $30,000 in saved medical costs. Weinberg has contributed to this program across eight states, serving nearly 4,500 older adults.

Through an interdisciplinary team of health professionals, PACE provides comprehensive medical and social services to older adults who qualify for nursing-home care, enabling over 95% to remain in their home. PACE has reduced hospitalizations, emergency room visits, and nursing home stays, while increasing longevity and quality of life.

Since 2017, the Foundation has invested in 10 affordable housing projects that provide supportive environments with services tailored to LGBTQ older adults. Weinberg’s partnership with SAGE has led to almost 1,000 new units of such housing. These developments also provide key services, including access to health care, as well as a true sense of community and belonging for LGBTQ older adults and their families.
