All Kids Play Individual Youth Sports Financial Assistance Grant Application is a grant from All Kids Play that provides financial assistance to low-income families for youth sports registration fees, equipment, uniforms, and other reasonable sports costs for children in grades K-12.
Families must have gross household income below 60% of the estimated state median income by household size to qualify for a full grant, or below the state median income for a 50% partial grant. Applications are accepted on a rolling, need and first-come basis. Families may apply up to four times per year per child for recreational sports, capped at $350 per sport per session and $3,500 annually per child.

All Kids Play Individual Youth Sports Financial Assistance Grant Application - All Kids Play All Kids Play Individual Youth Sports Financial Assistance Grant Application All Kids Play Individual Youth Sports Financial Assistance Grant Application Financial assistance is available to qualifying families on a need, first-come, first-served and funding availability basis to pay for the following: Youth sports registration fees Equipment and/or uniforms used for youth sports Other reasonable costs associated with youth sports Grants must be for children in grades K-12.
Must make less than the following household gross income amounts based on household size (based on 60% of the Estimated State Median Income by Household Size, Low Income Home Energy Assistance Program, U.S. Dept of Health and Human Services, May 29 2020 ).
For example, in the state of Illinois, must make less than the following income by household size: A family may apply up to 4 times per year for each child for recreational sports (capped at $350 per sport per session). May apply 1 time per year for travel and 2 times per year for recreational sports for each child (different sports than the travel sport). A family can generally only apply for a grant for one sport at a time per child.
All Kids Play reserves the right to evaluate overlapping grant applications and approve at its discretion, considering many sports seasons have at least some overlap. In general, a child should not be playing multiple sports at a time with our financial assistance. An application is required for each grant and for each child.
All Kids Play reserves the right to cap annual grant distribution to $3500 per child per family. Confidential grant applications are addressed on a needs and first-come, first-served basis as funds are available. All Kids Play reserves the right to make exceptions to eligibility guidelines for reasonable extenuating circumstances.
NOTE: Part of our program is to ensure a sports organization has the best interest of the athletes and their families in mind. To that point, we make every attempt to work with the sports program to ensure a safe and healthy environment and also to ensure they are putting in their best effort to support student-athletes.
As such, if the grant is for a travel, club, or elite sport, training academy, monthly on-going classes (such as gymnastics, martial arts, or competitive dance, etc.) or a for-profit youth sports organization, we request that the sports organization match our grant to cover 50% of the cost.

I hereby expressly waive and release any and all claims and disputes, now known or hereafter known in any jurisdiction throughout the world, against the Organization, and its officers, directors, employees, agents, affiliates, members, successors, and assigns (collectively, “Releasees”) arising out of or attributable to my participation in the Activities, whether arising out of the negligence of the Organization or any Releases or otherwise and I assume all risk associated with the Activities.
I covenant not to make or bring any such claim against the Organization or any other Releasee, and forever release and discharge the Organization and all other Releases from liability under such claims.
I shall defend, indemnify, and hold harmless the Releasees against any and all losses, damages, liabilities, deficiencies, claims, actions, judgments, settlements, interest, awards, penalties, fines, costs, or expenses of whatever kind, including reasonable attorney’s fees, fees and the costs of enforcing any right to indemnification under this Agreement, and the cost of pursuing any insurance providers, arising out of or resulting from any claim of a third party related to the Activities.
This Agreement constitutes the sole and entire agreement of the Organization and me with respect to the subject matter contained herein and supersedes all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, with respect to such subject matter.
If any term or provision of this Agreement is invalid, illegal, or unenforceable in any jurisdiction, such invalidity, illegality, or unenforceability shall not affect any other term or provision of this Agreement or invalidate or render unenforceable such term or provision in any other jurisdiction. This Agreement is binding on and shall insure to the benefit of the Organization and me and their respective successors and assigns.
All matters arising out of or relating to this Agreement shall be governed by and construed in accordance with the internal laws of the State of Illinois without giving effect to any choice or conflict of law provision or rule (whether of the State of Illinois or any other jurisdiction).
Any claim or cause of action arising under this Agreement may be brought only in the federal and state courts located in Cook County, Illinois and I hereby consent to the exclusive jurisdiction of such courts.

I have the legal right to consent to and, by signing below, I hereby do consent to the terms and conditions of this Release of Liability and Assumption of Risk.

BY SIGNING, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE TERMS OF THIS AGREEMENT AND THAT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE ORGANIZATION. Types of Information Collected We may collect personally identifiable information from you including, without limitation, non-public information about you that helps All Kids Play identify you, such as first and last name, birth date, business title, employer, email address, employer address, office and home telephone number, mobile telephone number, All Kids Play account username and password, unique device identifier of your devices that connect to or access the Sites, occupation, preferences, information regarding services you obtain from us, information you provide to us when you register in connection with our web or mobile applications, and other information that you provide to All Kids Play or that is generated through your use of the Sites or interactions with All Kids Play that are not otherwise publicly available.
We also may collect non-personally identifiable information about you, such as information regarding your use of the Sites, your IP address, Sites usage statistics, time stamps, browsing history, general geographic location, type of device you use to connect to the Sites, type of computer operating system you use (e.g., Microsoft Windows, Google Chrome OS, Mac OS, Android or iOS), the type of browser you use (e.g., Firefox, Chrome, Internet Explorer, Safari), the domain name of your internet service provider, and information regarding your Internet location when you see All Kids Play’s ads and whether you click on such ads.
We also may receive certain aggregated or anonymous information regarding users of the Sites such as the age, gender, occupation, zip code, interests, purchase preferences, and language preferences of users of the site, as well as information collected through questionnaires and surveys.
“Non-personally identifiable information” refers to information that does not personally identify the individual to whom the information relates, such as information that is aggregated by All Kids Play or a third party, or information that is not linked to contact information or other personally identifiable information of an individual.
In the event that any of the non-personally identifiable information that we collect about you is maintained, used or shared in a manner that associates it with your name, contact information or other personally identifiable information, it will be treated as personally identifiable information.
If you are logged into your account on the Sites, information such as the content you access, view, click on or search for when on the Sites, the devices from which you access such content and the length of time for which you view same, may become personally identifiable. In such an event, All Kids Play will treat such information as personally identifiable information. 3.
