The Family Assistance Program Grant provides financial assistance to individuals with dementia that require the assistance of a caregiver for daily functions.
The grant aims at assisting caregivers by making their lives easier. This $500 grant can be awarded to one care recipient (patient) twice per calendar year (January 1 to December 31) with a 90 day waiting period between uses. The care recipient (patient) must reside in Arkansas.
The care recipient (patient) must live independently or with family (not in a full-time care facility). The care recipient (patient) must require the assistance of a caregiver for daily functions (must be stated on the diagnosis). The care recipient (patient) must have an official diagnosis of any dementia on a physician’s official letterhead or prescription pad.
