At the time of the application, all Bureau grantees must submit for each grant program the following documents: Uniform Application for State Grant Assistance (pdf) CSA Budget/Tracking System and Registration Information Complete applications must be received by The Department at the specific location no later than 12:00PM CDT, May 5, 2025. The completed documents are to be emailed to the Bureau email address DHS. OFWNOFO@illinois.
