Capacity Building - Michigan Health Endowment Fund Capacity Building at the Health Fund HEALTH-FOCUSED NONPROFITS Nonprofits have a critical role in supporting people and communities across the state to improve their health. Every day, we see organizations working diligently for a healthier Michigan — often in the face of major constraints.
Our Capacity Building Initiative seeks to equip health-focused nonprofits with funding, tools, and training to level up their work and make progress toward their missions. To that end, we offer a range of options to help grantees and other organizations build on their strengths and grow their capacity.
The Capacity Building grant program assists health-focused, community-based organizations in becoming stronger, more effective institutions within their community, allowing them to spend more time and energy focused on their missions. Grants also support collaborative efforts among providers, service agencies, and community-based organizations to cooperatively and sustainably address health issues.
The Health Fund makes grants of up to $300,000 for both organizational and collaborative capacity building projects for health-focused organizations that support children and older adults, or that are aligned with our priority areas . Catchafire: Skilled volunteers looking to help you Eligibility: The Health Fund offers Catchafire access to all health focused nonprofits in Michigan, and other Michigan nonprofits might be eligible through the One Michigan portal. MemoryFox is a digital platform that helps nonprofits collect and share stories, photos, and videos that bring their impact to life.
Create testimonials from program participants, curate media libraries of your work in action, and share meaningful stories in fundraising campaigns, donor stewardship communications, impact reports, and more. The MemoryFox team provides technical assistance and hands-on guidance to help nonprofits develop tools to connect with their community.

Michigan Nonprofit Association technology partnership The Health Fund partners with MNA to provide technology assessments; funding of up to $5,000 for new equipment such as laptops, monitors, servers, and conferencing software; and up to two years of cybersecurity support. Eligibility: Participation requires an organizational technology assessment and depends on the size of the organization.
Bonterra Jumpstart program This immersive, yearlong program pairs organizations with a personal fundraising coach, who helps in acquiring, retaining, and upgrading individual donors. In addition to dedicated coaching, the Jumpstart program includes technology tools and technical assistance designed to help you execute strategies built with your coach, fundraise more effectively, and take advantage of heightened online giving.
Eligibility: Cohorts are offered annually. Organizations must complete an assessment to be eligible. PolicyMap provides data about Michigan residents that can help nonprofits communicate their impact and demonstrate need or access to services.

Accreditation Support Rural Michigan Dental Hygiene School Launch North Central Michigan College Foundation Program: Capacity Building Geography: Antrim, Charlevoix, Cheboygan, Emmet, Kalkaska To address severe dental workforce shortages in Northern Michigan, this project will help the college prepare internal systems and staff to meet accreditation standards for a new dental hygiene program.
By strengthening evaluation tools, attending national accreditation training, and completing required documentation, the college will be ready to admit its first students and launch a community clinic that expands preventive care access for over 8,000 residents.
Advancing Health Equity for 2-SLGBTQ+ Individuals in West Michigan Grand Rapids Pride Center Program: Capacity Building Geography: Allegan, Barry, Ionia, Kent, Montcalm, Muskegon, Newaygo, Oceana, Ottawa The Grand Rapids Pride Center will hire a front desk assistant, enabling staff to focus on health initiatives and improve service delivery for its Health Equity Program, which addresses health disparities faced by the 2-SLGBTQ+ community.

Affordable Housing in Pontiac Presbyterian Village North DBA The Village of Oakland Woods Program: Capacity Building Geography: Oakland The Village of Oakland Woods will hire temporary help to apply for Low-Income Housing Tax Credits, aiming to increase affordable senior housing and health care access.
Archaic to Modern: Organization Infrastructure Improvements AgeWell Services of West Michigan Program: Capacity Building Geography: Muskegon, Oceana, Ottawa AgeWell Services’ Discount Dine program, which offers healthy food at discounted rates while preserving dignity in access, serves older adults in their community.
To enhance accessibility, AgeWell Services will implement new technology and processes to streamline card issuance, eliminate wait times, and support additional processing locations, ultimately increasing capacity and enabling the organization to serve more older adults in the community.
Audit-Ready: Strengthening NCDM’s Financial Infrastructure at a Pivotal Juncture National Council of Dementia Minds Program: Capacity Building Geography: Statewide To support its growth and prepare for its first independent audit, this project will help the organization update accounting policies, transition to accrual-based reporting, and strengthen board oversight.
These improvements will increase transparency, ensure regulatory compliance, and position the organization to scale its peer-led dementia support programs nationally.
Building a Stronger Community-Service Agency Model Jewish Family Service of Metropolitan Detroit Program: Capacity Building Geography: Macomb, Oakland, Wayne Jewish Family Service of Metropolitan Detroit will enhance its care management services through Medicaid and Medicare billing for Community Health Workers (CHWs). The initiative aims for full credentialing for sustainable service delivery.

Capacity Building Medical Respite The Salvation Army Great Lakes Division Program: Capacity Building Geography: Statewide The Harbor Light program operates in multiple locations across Michigan, seeking to strengthen partnerships with hospitals by demonstrating how their programs reduce readmissions and healthcare expenses, particularly in preparation for upcoming Medicaid policy changes covering medical respite services.

Detroit Youth Mental Wellness & Resilience Collaborative Program: Capacity Building Geography: Wayne Detroit’s youth are facing escalating mental health challenges, with nearly half experiencing anxiety, depression, or trauma exacerbated by exposure to violence and poverty, along with limited access to services.
This project aims to bridge these gaps by empowering youth and service providers through training, community-based support, and the establishment of a Learning Community of Practice, which fosters collaboration and equips professionals with skills to better address mental health issues while also managing their own stress.
Developing CHW Capacity at JFS Jewish Family Services of Washtenaw County Program: Capacity Building Geography: Washtenaw Through this grant, JFS aims to enhance their CHW capacity by training and certifying four CHWs, focusing on Latino youth and families, adults with complex health needs exacerbated by social determinants of health, and vulnerable older adults, with the goal of extending reach, improving health outcomes, and facilitating Medicaid reimbursement for coordinated services to ensure sustainable support for the community.
Developing Internal Structural Capacity for Organization/Program Evaluation & Monitoring Program: Capacity Building Geography: Wayne To prepare for a leadership transition and strengthen internal learning, this project will build a centralized evaluation system that helps staff collect, organize, and use data across five core programs.
The system will include shared tools for survey collection, goal tracking, and real-time decision-making, while also training a staff member to serve as the organization’s first internal evaluator and embedding quarterly reflection sessions into team routines.

Empowering DCWs for Improved Health Outcomes Program: Capacity Building Geography: Statewide High turnover among direct care workers (DCWs) has created inconsistent care for individuals with intellectual and developmental disabilities (IDD) and older adults in foster care, necessitating targeted workforce solutions.
This project will conduct focus groups to identify factors contributing to job dissatisfaction and develop a pilot program aimed at improving retention and job satisfaction, ultimately enhancing care quality for vulnerable populations.
Enhancing Access, Inclusivity, and Efficacy in CHW Training Michigan Community Health Worker Alliance Program: Capacity Building Geography: Statewide Since 2011, MiCHWA has been instrumental in integrating community health workers (CHWs) into Michigan’s health systems, enhancing access to services and improving cultural competence.

Clair Butterfly Foundation Program: Capacity Building Geography: Benzie, Grand Traverse, Kalkaska, Leelanau To meet rising demand for trauma-informed education, this project will expand the training team, upgrade digital tools, and strengthen evaluation systems. These enhancements will equip educators, courts, and community partners with lasting resources to support youth resilience and emotional well-being.
Financial Development and Migration ShareCare of Leelanau, Inc. Program: Capacity Building Geography: Leelanau ShareCare provides essential services for older adults in Leelanau County and has seen an increase in demand for services such as transportation, prescription delivery, and caregiver respite.
Through this work, they will transition to a new online financial system, develop financial projections, and enhance financial management practices to improve stability and diversify funding sources.

Heath Equity Capacity Network for Latino and Asian Americans Michigan State University Program: Capacity Building Geography: Statewide This project aims to address the growing needs of older Asian and Latino Americans in Michigan, particularly concerning dementia and depression, by building a collaborative network among service providers and institutional leaders.
The project seeks to develop culturally relevant health care services through community-based research and expert panels, ultimately advancing health equity and improving outcomes for older adults and caregivers in these communities.
Housing as Health: A Data-Informed System Response Program: Capacity Building Geography: Kent Through this grant, Housing Kent plans to implement a predictive data system to proactively identify households at risk of eviction or homelessness, aiming to address root causes, prevent negative health impacts, and foster community resilience through comprehensive collaboration and data-driven intervention strategies.
How YOU Birth Doula Billing Buddies Program: Capacity Building Geography: Statewide Access Health, Inc. will support a doula cohort in West Michigan by training them on Medicaid billing processes. The program will include a peer-instructed training model, “Billing Buddies,” to enhance participation and sustainability for doulas facing challenges and frustrations with credentialing.
Implementation of Supported and Transitional Employment Program: Capacity Building Geography: Oakland This grant will facilitate the hiring and certification of transitional and supported employment specialists, enabling Gesher to apply for Clubhouse International accreditation, bill their local CMH organization for Clubhouse services, and ultimately improve health outcomes for individuals with severe and persistent mental illness by addressing social determinants of health through employment opportunities.
Implementing an Organizational Operating System Program: Capacity Building Geography: Kent AYA Youth Collective supports youth aged 14-24 at higher risk of homelessness, particularly those identifying as LGBTQIA+, people of color, and those with involvement in foster care or juvenile justice, providing 24 supportive housing units across nine homes in Kent County.
To facilitate their planned expansion to 80 housing units by 2026, AYA will implement an Entrepreneurial Operating System (EOS) to establish structured processes and policies, enhancing productivity, accountability, and data-driven decision-making for sustainable growth.
Increasing Prevention Programming Capacity Children's Assessment Center Program: Capacity Building Geography: Allegan, Ottawa To expand its community education efforts, this project will implement Coalition Manager, a cloud-based platform that automates registration, certification, and reporting while consolidating five separate systems.

Integrated Relationship Management System Program: Capacity Building Geography: Allegan, Ottawa Community Action House (CAH) has significantly expanded its staffing, volunteer numbers, and operating budget over the past five years, enhancing its core services, particularly through its Food Club, which provides low-income families with access to fresh, healthy foods.
To sustain this growth and further improve its impact, grant funding will be used to upgrade and integrate donor and volunteer management systems into a single platform, streamlining operations and strengthening capacity for future initiatives. a MiGen Program: Capacity Building Geography: Statewide Through this grant, MiGen aims to enhance its evaluation practices, engage a DEI consultant to ensure inclusivity, and provide leadership training for staff, ultimately bolstering organizational capacity, improving program effectiveness, and fostering equitable and sustainable growth.
KSSN: Professional Learning and Development Kent School Services Network Program: Capacity Building Geography: Kent In the 2024-2025 school year, KSSN will have 48 Community School Coordinators across 47 schools, focusing on connecting students and families to essential services like Medicaid and food assistance to meet diverse needs and reduce health disparities.

Meals on Wheels Program Capacity Building Jewish Family Services of Washtenaw County Program: Capacity Building Geography: Washtenaw Jewish Family Services of Washtenaw County aims to increase meal production for home-delivered meals, supported by temperature monitoring and recipe management software.
Modeling Sustainable and Scalable Integrated Team Care Benzie Leelanau District Health Department Program: Capacity Building Geography: Benzie, Grand Traverse, Leelanau Following the success of a 2023 Health Fund grant, which significantly increased caseloads for CHWs due to integrated data between healthcare partners, this grant will expand a care coordination network to meet the rising demand exacerbated by increased accreditation requirements, including mandatory Social Determinants of Health screenings for admitted patients.
