Thank you for your patience, and we hope to continue to serve the Carolinas and partner with our grassroots nonprofits in the coming year. As a values-driven institution, Constellation Quality Health is committed to giving back to the communities where it does its work. We advance this goal through our charitable foundation, established in 2018 and named in honor of the organization’s first 40 years of existence – the CCME Foundation.

The CCME Foundation works to improve the health of individuals and communities. We provide charitable grants to nonprofit organizations to build upon or undertake new projects that improve access to health care and health-related services for our neighbors in the Carolinas. Funding is prioritized in accordance with the Foundation’s recently adopted Promising Practices Framework .

We welcome all eligible applicants, especially community-based organizations in earlier stages of development. Since its founding, the CCME Foundation has provided support for healthy lifestyle and quality of life programs, access to culturally and linguistically appropriate care, health-related educational outreach, consumer-centered care and decision-making, and patient safety programs.

Since 2021, the CCME Foundation has awarded more than $1. 5 Million in grants to organizations in the Carolinas that have made a big impact through their community-based programs and services. Check out the 2025 Impact Report .

Explore the difference we are making through these stories of impact: Caregiver Mental Wellness Cabarrus Public Health Interest The Carolinas Foundation for Hospice & Home Care Dementia Alliance of North Carolina Greenville Free Medical Clinic The Hunger and Health Coalition Western Carolina Medical Society The CCME Foundation provides charitable grants to nonprofit organizations in North Carolina and South Carolina seeking to improve health quality and equity.

Grants range from $5,000 to 50,000 per organization. We prioritize projects that focus on measurable outcome improvements and have a high impact on health care improvements and access, particularly for uninsured and underserved populations. A focus on improved outcomes and access to health care is essential.

In a constantly evolving health care landscape, this often requires new approaches and innovations.

The CCME Foundation is implementing its Promising Practices Framework to encourage partnership and innovation at the grassroots level primarily in rural and sparsely suburban populations and/or in under and unserved populations within the following priority focus areas: post-acute care, behavioral health, chronic disease management, and workforce development.

Please note: this does not mean that the CCME Foundation will not fund urban community projects; urban-based organizations are also encouraged to apply.
