Applicants use this form when they have completed the process of obtaining a CDA or renewal and would like reimbursement for their expenses. To successfully submit your form, read the documentation requirements and the questions listed below. Gather required documents before filling out the form.

Throughout its work, IMLS places importance on diversity, equity, and inclusion. The California Department of Education (CDE) Early Education Division is making approximately .7 million available to expand California State Preschool Program (CSPP) services statewide, appropriated under the 2021 Budget Act.
