Good & Healthy Community Grant Good & Healthy Community Grant The Office of Chronic Disease Prevention and Health Promotion (OCDPHP) within the SD Department of Health (SD DOH) is committed to helping communities in South Dakota implement evidence-based chronic disease prevention and health promotion strategies to improve where they live, work, learn and play.
The Good & Healthy Community Grant is designed to support local community and organization-based efforts, foster collaboration among organizations, and support the mission of the SD DOH, which is to promote, protect and improve the health of every South Dakotan.
Anticipated Award Amount: $5,000 minimum; $125,000 maximum Grant Release: February 1, 2021 Submission Deadline: NOW CLOSED Project Period: June 1, 2021—May 31, 2023 Grant Application Information A community health needs assessment (CHNA) is required of all applicants. Please reference the Grant Guidance document below for detailed information on CHNA and other application requirements.
