However, the Department of Children and Youth has elected to establish a Wait List should additional funding become available at a later time. Please be advised that the Wait List is for informational purposes only and DOES NOT guarantee that further funding will be provided. This Wait List is only open to Community and Faith-Based organizations in Ohio.
If your organization wishes to be included on the Wait List, please include the following information: A. Applicant must be a current not-for-profit 501(c) organization and shall submit a copy of the tax status determination letter from the IRS and filed with the Ohio Secretary of State. B.
Applicant must complete, sign, and submit the Standard Affirmation and Disclosure Form, in accordance with the Governor’s Executive Order 2022-02D. C. Applicant must complete all sections of the Application, without any changes to the terms and conditions.
Any application that does not include all required sections may be deemed incomplete. Applications to be on the Wait List are to be submitted to Procurement_Inquiries@childrenandyouth. ohio.
