Community Benefit Program is sponsored by UnityPoint Health. As a not-for-profit healthcare system, UnityPoint Health invests in its communities to improve health, particularly for vulnerable populations.

How to Request a Community Sponsorship We’re proud to support community organizations, projects, activities and initiatives that improve the health, economic and social well-being of the people we serve. As a not-for-profit organization, we believe in the value of giving back to our communities.

It's our intention to contribute resources to activities that improve community health and assist community social service organizations, and other non-profit organizations' abilities, to enhance the quality of life in our communities.

Community Sponsorship Request Guidelines We’ve developed guidelines for requests for community sponsorships, cash contributions and in-kind donations to ensure the support from UnityPoint Health is consistent with our mission to improve the health of the people and communities we serve.

The following requests will not be considered: Solicitations from for-profit organizations Political campaigns, candidates, parties or partisan activities Sectarian, denominational or religious organizations for support of theological functions Fellowships or internships Labor or fraternal groups Community Sponsorship Requests by Location UnityPoint Health – Cedar Rapids All organizations seeking a contribution should complete the Community Contribution Request Form .

Requests will be reviewed, and applicants will be notified within 30 days via email if the contribution being requested is approved or denied. Requests will not be considered for programs outside of the Cedar Rapids region. UnityPoint Health – St.

Luke's Hospital UnityPoint Health – Des Moines All organizations seeking a contribution should complete the Community Contribution Request Form . Requests will be reviewed, and applicants will be notified within 30 days via email if the contribution being requested is approved or denied. Requests will not be considered for programs outside of central Iowa.

UnityPoint Health – Des Moines 1200 Pleasant Street - Administration UnityPoint Health – Fort Dodge All organizations seeking a contribution should complete the Community Contribution Request Form . Requests will be reviewed and awarded on a quarterly basis. Notification will be sent by email or mail to all organizations submitting a request for the quarter.

If your request is not granted, it can be rolled over to the next quarter without the need to re-submit the paperwork. Requests will not be considered for programs outside of the Fort Dodge region. UnityPoint Health – Fort Dodge UnityPoint Health – Grinnell All organizations seeking a contribution should complete the Community Contribution Request Form .

Requests will be reviewed, and applicants will be notified within two to four weeks by email. Requests will not be considered for programs outside of the Grinnell region. UnityPoint Health – Grinnell Regional Medical Center UnityPoint Health – Quad Cities 2027 Sponsorship Applications The 2027 UnityPoint Health – Quad Cities community sponsorship application process is by invitation only.

Organizations invited to apply will receive an email from the Trinity Health Foundation no later than August 1, 2026. We will consider one sponsorship per organization/group per year. If an organization submits more than one proposal, we will choose the one that best fits with our overall goals/priority funding areas.

Requests for in-kind and medical sponsorships must also be submitted through the online application process. If your organization does not receive an invitation to apply this year but you feel your mission/project aligns with UnityPoint Health – Quad Cities’ priority funding areas below, please complete our Community Sponsorship Intake Form . Our team will review it before the next application cycle.

Requests for sponsorships will be reviewed and applicants will be notified within 45 days of receipt via email and/or in writing whether the contribution being requested is approved or denied.

One sponsorship per organization/group will be considered per calendar year for entities located in the Cedar Valley. Requests for in-kind must also be submitted through the online application process.
