Community Change Grant Program (2026) is sponsored by America Walks (in partnership with CDC's Division of Nutrition, Physical Activity, and Obesity). This program awards grants for projects focused on creating healthy, active, and engaged places to live, work, and play, supporting the CDC's Active People, Healthy Nation℠ initiative.
It aims to provide support to local organizations to advance safe and enjoyable places to walk and move, funding projects that demonstrate increased physical activity and active transportation in a specific community, engage new people and organizations, and demonstrate a culture of health and design.
