Community Dividend Fund is sponsored by Sanford Health. Sanford Health invests in initiatives that strengthen community health and improve quality of life. Eligible nonprofit organizations addressing demonstrated community health needs may receive financial support.
Funding priorities are informed by Sanford's Community Health Needs Assessment (CHNA), strategic focus areas, and emerging local community needs. Categories include Healthcare Access and Outcomes, Neighborhood and Infrastructure Advancement, Priority Populations and Special Community Projects, Social and Community Wellbeing, and Workforce and Education.

Sanford Health publishes an annual Community Impact report to share key programs and services that have been implemented across our footprint to meet the needs of our communities and improve the overall health and well-being of those we serve.

The Community Health Needs Assessment allows us to identify significant unmet health needs and prioritize local partnerships and initiatives.

Investments that make a difference Every community is unique, so we consider local conditions for each funding request. We have established criteria to guide these decisions. Read funding request criteria.
Charitable donations, sponsorships and community investments Sanford Health regional offices review funding requests as they are received. Funding requests must demonstrate an identified community health need. Initiatives that support community health priorities may take precedence.
Please allow up to six weeks for your application to be reviewed. Additional processing time may be necessary if you are a new vendor or additional information is needed to process payment. Please note that Sanford Health can’t accept funding requests from: Political campaigns and candidates Individuals requesting personal funding Questions?
