This 2026 grant cycle will provide funding in various amounts for this year only and will be eligible to organizations in Polk County. The grant aims to enhance community well-being, address population- specific health challenges, and improve access to high-quality healthcare services.

In the 2026 funding cycle, AdventHealth Heart of Florida and AdventHealth Lake Wales have prioritized Mental Health, Health Care Access and Quality, and Nutrition and Healthy Eating, as the key areas of focus for this grant.

These areas of focus were the most pressing health needs in the Polk County community, as identified in our 2025 Community Health Needs Assessments that can be accessed here: 2025 AdventHealth Heart of Florida Community Health Needs Assessment 2025 AdventHealth Lake Wales Community Health Needs Assessment To be eligible for the grant an organization must: • Provide programmatic services in Polk County within the areas of Health Care Access and Quality, Mental Health, or Nutrition and Healthy Eating.

Note: Organizations serving communities around AdventHealth Heart of Florida and AdventHealth Lake Wales will be given priority. • Submit a recent W-9. • Demonstrate self-sustainability of the organization and plans for sustaining the program or project, including additional funding or revenue sources, provided the organization does not receive grant funding from AdventHealth in subsequent years.

• Present meaningful impact and progress through metrics by providing data to AdventHealth on a quarterly basis once funds have been received and the program has begun. • Indicate partnerships and collaborations needed to reach program goals. • Serve community members residing in Polk County.

• Provide a contact person, along with their email address and phone number, who will be notified of the application status. • Agree to sign a Memorandum of Understanding (MOU) between AdventHealth and your organization if awarded funds for the project or program. • The 2026 Community Impact Grant will be open for applicants March 1, 2026 through June 1, 2026.

• Once the application closes on June 1, 2026, no other submissions will be accepted for grant funds. Note: If additional funds are identified after the grant funds have been distributed to the awardee(s) in the 2026 cycle, the application will reopen for more submissions.

• Applications will be reviewed by a committee, and applicants will be notified of the status of their submission, for both those who are awarded funds and those who were not, on August 3, 2026. • Funds will be awarded to approved applicants by October 1, 2026.
