Community Micro-Grant Program is sponsored by Forge AHEAD Center (funded by National Institute on Minority Health and Health Disparities - NIMHD). This program supports community organizations in Alabama, Louisiana, and Mississippi working to reduce health disparities in diabetes, obesity, and hypertension. It aims to empower community-led projects that are innovative, collaborative, and sustainable.

Apply for Community Micro-Grant Funding | Forge AHEAD Center | Deep South USA Community Micro-Grant Program Community Micro-Grant Program The Forge AHEAD Center is excited to launch the Community Micro-Grant Program , designed to support community organizations in Alabama, Louisiana, and Mississippi that are working to reduce differences in health outcomes across populations in diabetes, obesity, and hypertension .

This program is part of our Community Engagement Core (CEC) and is funded by the National Institute on Minority Health and Health Disparities (NIMHD) under award P50MD017338 . We seek to empower community-led projects that are innovative, collaborative, and sustainable. We welcome applications from organizations that: Are registered 501(c)(3) nonprofits located in Alabama, Louisiana, or Mississippi.

Have a community-focused mission that aligns with health promotion, chronic disease prevention, or reducing gaps in health access and outcomes. Can implement a 12-month project that directly benefits populations experiencing differences in health outcomes across populations. Demonstrate the ability to manage grant funds and track outcomes effectively.

Note: Organizations currently receiving funding from the Forge AHEAD Center or its sub-awardees are not eligible to apply.

Grant Amount: Up to $10,000 per project Project Duration: 12 months (must be completed by June 30, 2026 ) Training specific to the project Not Eligible: Permanent equipment (unless justified), lobbying, political advocacy, alcohol purchases, general operating expenses for organizations with revenue over $100K, or indirect costs presented as percentages. Applications will be evaluated using the following criteria: Alignment – How well does the project address community health needs? Feasibility – Are the goals, timeline, and budget realistic? Innovation – Does the project offer a creative and effective solution?

Collaboration – Does the project engage multiple stakeholders ? Sustainability – Can the initiative continue beyond the funding period ? Expectations for Grant Recipients Grant recipients will be required to: Attend monthly support meetings with the Forge AHEAD Community Engagement Core .

Participate in a mid-year progress check-in . Submit progress and financial reports , including a final impact summary . Document and share project outcomes through photos, videos, or testimonials.

Present achievements at a Community Partner Showcase in their state. Acknowledge the Forge AHEAD Center in all project-related communications. Project Completion Deadline Frequently Asked Questions (FAQs) How many grants will be awarded?

We anticipate awarding multiple grants of up to $10,000 each. The total number of awards depends on the number of high-quality applications received. Can organizations submit more than one proposal?

No. Each organization may submit only one proposal per funding cycle . What type of projects are most competitive? Projects that demonstrate strong community partnerships, sustainability beyond the funding period, and measurable impact will be prioritized.
