Quintero, Secretary IDHS Grants Administration Family & Community Services Grants - FY 2026 Office of Community & Positive Youth Development Opportunities Community Wellness (26-444-80-1489) Continuation Application Illinois Department of Human Services Division of Family and Community Services Funding Opportunity Title Funding Opportunity Number Catalog of State Financial Assistance (CSFA) Number Catalog of State Financial Assistance (CSFA) Popular Name Catalog of Federal Domestic Assistance (CFDA) Number(s) The Community Wellness In-Home Visitation Project/Good Neighbor Campaign will provide coordinated nonclinical, in-home wellness visits, case management, and crisis intervention services, as well as make referrals for wraparound support services to the target population.
The populations targeted include Seniors, Single parent households, 1st time pregnant females, victims of domestic violence, Returning Citizens to the community (Adults and youth returning from secure confinement), and high risk disengaged youth (youth ages 16-24 that are not employed or enrolled in school), and low income family/households.
Those eligible for services must also be a resident of Chicago living within a 50-block radius of zip code 60651, 60624, 60639 and 60644. This program seeks to ensure that the health and social needs of these at-risk individuals are being adequately addressed by qualified health and social service providers and are connected to their communities.
FY26 Continuation Application Current Providers will receive a continuation application packet via email. Providers may request additional copies of the continuation application by sending an email to DHS. YouthServicesInfo@illinois.
gov This continuation application is for Fiscal Year 2026 funding for Community Wellness (444-80-1489) Eligibility is limited to current Community Wellness providers funded under CSFA# 25-444-80-1489. Each applicant must be registered in the Illinois GATA Grantee Portal and must continue to be GATA Pre-Qualified . Applicant organizations must complete the FY26 ICQ before an award can be made.
(Accessed through the Grantee Portal.) If the FY26 ICQ is not available in the Grantee Portal prior to the application due date, grantees should submit their application without the ICQ and will be notified when it becomes available. Applicant organizations must submit a complete FY26 Application Packet by the due date listed in the continuation application to DHS.
