Baltimore City Continuum of Care | Baltimore City Error Enter a word or phrase to search The Baltimore City Continuum of Care (CoC) is a U.S. Department of Housing and Urban Development (HUD) initiative that includes organizations that serve people experiencing homelessness, persons with lived experiences of homelessness, and other stakeholders with a passion for ending homelessness.
The CoC carries out responsibilities assigned by HUD's CoC Program and coordinates available resources and stakeholders' efforts to make homelessness rare, brief, and non-recurring in Baltimore City. Baltimore City CoC Leadership Jamal Turner , Vice Chair Ernestina Simmons, LCSW-C , Lead Agency Representative The Mayor's Office of Homeless Services (MOHS) is the designated lead agency for the CoC.
MOHS is responsible for implementing Federal, State, and local policy; promoting best practices, and administering and monitoring homeless services grants. MOHS provides approximately $48 million annually for homeless diversion programs. As the Collaborative Applicant and Lead Agency for the CoC, MOHS fulfills the following roles: Submit the annual CoC Consolidated Funding Application Apply for a grant for Continuum of Care planning.
