Applicants will be notified of their award status the week of July 6-10, 2026. Applications for the next funding period will open on March 1, 2027. MyMichigan Health is committed to helping our communities and reinforcing our purpose in ‘Creating Healthy Communities - Together.

’ One of the ways in which we do this is through our corporate giving program. This initiative was designed to award community benefit grants and sponsorships to eligible organizations for programs and projects that have a positive impact on health outcomes in each of the communities we serve. To manage the growing requests for support, MyMichigan Health has a **one-time, annual submission deadline**.

If you have a request that meets our criteria (listed below), please make sure to plan ahead for this annual application. ### Important Dates to Remember **Week of July 6-10, 2026**: Applicants who applied for Fiscal Year 2027 support will be notified of award status. **March 1, 2027**: Online application opens for Fiscal Year 2028 requests for support.

**June 1, 2027**: Application deadline for Fiscal Year 2028 requests for support. *Fiscal Year 2027 = July 1, 2026 - June 30, 2027 ### Qualifications for Funding Our Community Benefit grants process provides MyMichigan Health the opportunity to respond to requests and partner with non-profit organizations whose work is aligned with our Community Benefit priority areas.

To qualify for funding, your organization’s program must meet the criteria outlined below and address an identified health need within one of the following three funding priorities in our most recent Community Health Needs Assessment (2025).

This assessment is conducted every three years to which the areas of focus now through 2028 are the following: ### Focus Areas Focus Areas * **Chronic Disease Prevention** * **Access to Care & Social Services** * Social Determinants of Health ### Criteria for Support Criteria for Support Requests for funding must fit into the criteria below and aligned with our Community Benefit priority areas: 1.

Must be a qualified community health benefit or health-related community awareness or strategic direction activity. A community health improvement service should meet one of the following criteria: * Improve access to care. * Enhance public/population health.

* Advance knowledge through education and research. * Relieve government of burden for health. 2.

Promotes a culture of health. 3. Provides a direct and measurable impact on the health of the community.

4. Must be an IRS 501(c)(3) tax-exempt, non-profit organization. 5.

Meets one or more of the following community benefit/building objectives: * Target poor, medically underserved or special population. * Supplies a service that would be discontinued if the decision was made on a purely financial basis. * Accessible to the entire community regardless of their ability to pay.

Stimulates external community partnerships. * Improves access to care for underserved or at-risk populations.

### Ineligible Requests Ineligible Requests MyMichigan Health will not provide funding for the following: * Political campaigns/candidates * For-profit organizations * Activities or organizations outside the communities served by MyMichigan Health * Operating expenses (salaries, rent, utilities, maintenance of item, etc.) * Event operations (food, beverage, decorations, facility rental, etc.) * Travel expenses or conference fees * Organizations part of quid pro quo arrangement * School sporting teams, events, clubs * School yearbook advertising ### Awardee Expectations Awardee Expectations In order to comply with reporting requirements outlined in IRS Form 990 Schedule H, organizations receiving support are required to submit a progress report following the completion of the program or activity.

The report must be submitted 30 days after the event or 30 days post the one-year notification of the sponsorship for year-long programming. The report questions will be sent within the approval notification email. Those organizations that do not submit a report will be ineligible for future funding.

### How to Request Funds How to Request Funds While applications may be completed now for the next giving cycle, completed forms will not be reviewed until after the application deadline of June 1, 2026. Applicants will then be notified of their award status the week of July 6, 2026. Complete the online application.
