Application/Can We Help You? - Crozat Family Foundation Hello and thank you for taking the time to nominate yourself or a loved one. We receive approximately 100 to 150 applications and letters of nomination a month.

We wish we could contact and help everyone, however, our resources are limited, so we contact all of whom we believe will benefit most from reliable transportation, and we strive to serve two families a month. Program Requirements for Qualification : Follow-up documentation will be requested. One would need to have a current and valid California driver’s license.

One would need to be a resident of Sonoma County. One would need a verifiable source of income. Items needed to complete Application: An address within Sonoma County (Morgan street counts as an address) Letter of nomination: What to include The letter of nomination should include who, what, when, where and why for one’s need of reliable transportation.

Please include any information that may be pertinent to your situation, this may include but is not limited to custody arrangements, court dates, employment changes, housing issues, job loss, and/or unexpected expenses.
