CVS Health Foundation Grants | CVS Health CVS Health Foundation grants Grants are awarded to free and charitable clinics, school-based health centers and community health centers nationwide to increase access to care and improve health outcomes for people of all ages. Application for a CVS Health Foundation Grant is by invitation only.

The CVS Health Foundation has made a multi-million commitment to expanding access to quality health care nationwide through partnerships with the National Association of Free & Charitable Clinics (NAFC) and the National Association of Community Health Centers (NACHC).

In 2016, grants have been awarded to free and charitable clinics and community health centers nationwide to increase access, improve health outcomes and lower overall health costs across the country in innovative and strategic ways.

CVS Health Foundation $5 Million Community Health Investment Improves Access, Affordability National Association of Community Health Centers (NACHC) For the fourth year, we are partnering with NACHC and its members to find innovative solutions to chronic disease management through the "Innovations in Community Health" grant program.

We have awarded more than $3 million to support community-based health care models in community health centers across the United States. Learn more about NACHC and our grantees National Association of Free and Charitable Clinics (NAFC) For the third year, we are partnering with the NAFC, an organization that supports the needs of free and charitable clinics nationwide, to expand capacity and provide quality care.

While doctors and nurses volunteer their time and expertise at local clinics, staff and partnerships with other organizations in the community are critical for patients to achieve better health outcomes. Nearly $5 million in grant funding from the CVS Health Foundation will help the clinics to serve those in greatest need in their local communities.

Learn more about NAFC and our grantees Volunteer challenge grants Each year, CVS Health colleagues volunteer thousands of hours at nonprofit organizations in their communities. To help our colleagues lend even more support to the causes they care about, the CVS Health Foundation offers grants for volunteer service. Grants, which range from $500 to $5,000, are awarded directly to the organization on the colleague’s behalf.

Volunteer Challenge Grants are awarded on behalf of individuals who volunteer 15, 25 or 100 or more hours of service, to teams that volunteer more than 50 hours of service, and to both individuals and teams who fundraise for a charitable run, walk or ride event.
