gov contact center: Self-service knowledge base System for award management (SAM): The Delta States Rural Development Network Program supports the planning, development, and implementation of integrated healthcare networks within the Delta Region that collaborate to achieve efficiencies; expand access to, coordinate, and improve the quality of basic healthcare services and associated health outcomes; and strengthen the rural healthcare system as a whole.
Expanding access to care resources in the designated Mississippi Delta counties/parishes Utilizing evidence-based or promising practice models known to improve health outcomes and enhance the delivery Collaboration with network partners in the planning and delivery of healthcare services to increase access to care and reduce chronic disease Implementation of sustainable healthcare programs that improve population health, health outcomes, and demonstrate value to the local rural communities Applicants are required to propose projects focused on: Chronic lower respiratory disease Unintentional injury/substance use All award recipients are required to adopt an evidence-based or promising practice approach that proves demonstrated outcomes and may be replicable in other communities.
In addition, applicants are encouraged to incorporate value-based care (VBC) models or reimbursement strategies where appropriate.
Implement the project in an eligible rural area within the 8 Mississippi Delta Have demonstrated experience serving, or capacity to serve, rural populations with limited access to care within the 8 Mississippi Delta Region States Represent a network that provides services within a Have not previously received a Delta grant for the same or similar project unless the applicant is proposing to expand the scope of the project or the area that will be served through the project or add additional network Eligible applicants include State, county, city, township, and special district Independent school districts Public, private, and state controlled institutions of Federally recognized tribal governments Public and Indian housing authorities Native American tribal organizations Nonprofits with or without 501(c)(3) status For-profit organizations and small businesses HRSA expects to make 1 award per geographic service area as discussed in the application instructions.
The 8 Delta States: Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee Estimated award amount: $56,604 per eligible county/parish per year Estimated number of awards: 12 Estimated total program funding: Awardees will also receive technical assistance (TA).
Targeted TA will assist award recipients with achieving desired project outcomes, sustainability, and strategic planning, and will ensure alignment of the awarded project with the Delta Program goals.
