Family Assistance Program Grant Skip to primary navigation Family Assistance Program Grant (CLOSED) The Family Assistance Program Grant is funded through and administered by Alzheimer’s Arkansas and may be used by caregivers caring for a care recipient with a form of Dementia or chronic illness that is over 60 that requires assistance to perform daily functions.
Under the care recipient’s household funding may be utilized for, but is not limited to, the following: urgent needs, legal counseling, home care supplies, minor home modifications, mental health counseling, utilities, lawn maintenance, home cleaning, etc. Family Assistance Program Grant Details The care recipient (patient) must: • Live independently or with family (not in a full-time care facility) • Require the assistance of a caregiver for daily functions (must be stated in the letter of diagnosis) • Have an official diagnosis of any dementia at any age or a chronic illness over the age of 60 on a physician’s official letterhead or prescription pad All information on the application must be completed, signed, and include a letter of diagnosis of dementia (or chronic illness aged 60 or over) on official letterhead or prescription pad.
It may take up to 10 business days to process your application.
Once approved, grant funds may be used for the following items: • Caregiver mental health counseling • Minor home modifications (with prior approval) • Utility bills ( only : gas, electric, water/sewage, garbage) • Other services to reduce caregiver stress (i.e. home cleaning, cooking, yard work, etc.) Once approved, grant funds will be paid in full via check received with the approval letter.
After receipt of the grant check and funds have been spent, applicants must submit a Follow-Up Survey Log no later than 90 days after the approval date. ***Failure to return grant log and surveys will result in the disqualification of any future Alzheimer’s Arkansas grants.
