The Donated Dental Services (DDS) program in California provides comprehensive dental treatment to individuals who cannot afford dental care. The program relies on volunteer dentists and laboratories to offer services at no cost to eligible patients.

Please fill out the entire application to the best of your ability. Do not leave any sections blank and submit DDS application when you're finished. You will be placed on the waitlist, in the order your application was received, if you appear to meet general eligibility requirements (this does not guarantee that you will be approved for services).

You will receive notification that your application was placed on the waitlist within a month of submitting. If you are not eligible, we will send you a denial notification with alternative resources that may be able to assist. Depending on where you live, the waitlist can be several months to a year or more.

State DDS Coordinators are unable to give wait list estimates and cannot return phone calls about where you are on the wait list. When your application comes to the top of the wait list, a state DDS Coordinator will contact you to view your application. If you are eligible, you will be referred to a volunteer dentist for an initial consultation.

Final acceptance into the program will be made after the first consultation appointment with the dentist.
