We offer a range of opportunities to empower these individuals and help them achieve sustainable solutions. Additionally, we facilitate funding options for service providers, community partners, and developers to work together and create lasting solutions for the homeless population. Our ultimate goal is to revitalize our state and empower our citizens.

The Section 811 program allows persons with disabilities to live as independently as possible in the community by subsidizing rental housing opportunities that provide access to appropriate supportive services.

Emergency Solutions Grant Program The ESG program provides funding to: (1)engage homeless individuals and families living on the street; (2) improve the number and quality of emergency shelters for homeless individuals and families; (3) help operate these shelters; (4) provide essential services to shelter residents, (5) rapidly rehouse homeless individuals and families, and (6) prevent families/individuals from becoming homeless.

This program is designed to assist in the recovery of regions impacted by the disasters of 2020 and 2021. The program will offer eligible participants case management and housing subsidy to assist with obtaining and maintaining permanent housing.

It is designed to help households establish a foundation to support long-term permanent housing success. Continuum of Care Planning The goal of the CoC planning process is to create an annual plan to end homelessness. The CoC process involves compiling information about homelessness in the community, including information about homeless populations and inventories of homeless assistance resources.

This information is used to develop a list of priorities for funding, which helps determine how much funding a community will receive and for what projects.

Continuum of Care Coordinated Entry Coordinated entry is an important process through which people experiencing or at risk of experiencing homelessness can access the crisis response system in a streamlined way, have their strengths and needs quickly assessed, and quickly connect to appropriate, tailored housing and mainstream services within the community or designated region.

Standardized assessment tools and practices used within local coordinated assessment processes take into account the unique needs of children and their families as well as youth. SSI/SSDI Outreach, Access, and Recovery (SOAR) is a SAMSHSA endorsed case management mode designed to increase access to disability income benefits for individuals who are experiencing or at risk of homelessness.
