Food Insecurity Grant

Food Insecurity Grant is sponsored by Foundation for Health Equity (f4HE). Focuses on combating food insecurity in Northwest Philadelphia by supporting tax-exempt organizations. Grants aim to address immediate needs and systemic inequities, funding programs that provide food assistance and promote healthy aging.

Community Resiliency & Disaster Relief Community Action Leadership Elevance Health Foundation supports the efforts of community-based nonprofit organizations across the country that are improving lives and communities. As a catalyst for positive change, Elevance Health Foundation drives innovative and scalable solutions to address physical, behavioral, and social needs.

Over the next five years, the Foundation will focus our grantmaking efforts on partnerships and programs that can demonstrate measurable and positive change in four key focus areas: Community Resiliency & Disaster Relief The grant application process for each focus area requests specific information related to that area. The Foundation partners with national and local community-based organizations in advance of, during, and after disasters.) Anchored by our commitment to improving the health of the socially vulnerable, we are prioritizing partnerships and programs that address the physical, behavioral, and social needs of under-resourced communities.

Our grant ranges are open; we determine funding levels based on several factors, including the need and the plan presented to address it. Generally, our grants last from one to three years.

The Foundation will prioritize funding for programs that improve maternal/infant health, target disparities, address social needs, and remove barriers to care throughout the pregnancy journey—from pre-conception support to postnatal care for mothers and babies. The programs should drive specific, measurable maternal/infant health outcomes.

Elevance Health Foundation will consider proposals from qualified nonprofit organizations across the U.S. with an emphasis on: National programs. Promoting scalable and sustainable systemic change. Local programs.

Implementing relevant interventions in California, Florida, Georgia, Indiana, Missouri, Nevada, New York, Ohio, Texas, and Virginia.

Reducing disparities in pre-term birth rates Reducing disparities in severe maternal morbidity (SMM) incidence Improving maternal/infant health outcomes Download the Maternal/Infant Health RFP Grant applications will be accepted again in 2027 The Foundation will prioritize funding for programs that improve clinical health outcomes for individuals with diet-related conditions and increase access to nutritious foods for populations experiencing food/nutrition insecurity.

Elevance Health will consider proposals from qualified organizations across the U.S. with an emphasis on: National programs. Promoting scalable and sustainable systemic change. Local programs.

Implementing relevant interventions in California, Florida, Georgia, Indiana, Missouri, Nevada, New York, Ohio, Texas, and Virginia. Improving clinical outcomes for individuals with diet-related conditions experiencing high social risk Increasing access to nutritious foods for populations experiencing high social risk and food/nutrition insecurity Download the Food as Medicine RFP Grant applications will be accepted again in 2027.

The Foundation will prioritize funding for programs that increase the number of individuals receiving treatment for mental health (MH) and/or substance use disorders (SUD), support prevention and early-intervention strategies, and seek to reduce loneliness among individuals with these disorders. Elevance Health will consider proposals from qualified organizations across the U.S. with an emphasis on: National programs.

Promoting scalable and sustainable systemic change. Local programs. Implementing relevant interventions in California, Florida, Georgia, Indiana, Missouri, Nevada, New York, Ohio, Texas, and Virginia.
