Federally Qualified Health Center (FQHC) Incubator Project The Nevada Department of Health and Human Service (DHHS) administers the Federally Qualified Health Center (FQHC) Incubator Project, which is intended to create or enhance existing FQHC programs and services. DHHS, with approval from the 2017 Nevada Legislature, utilized allocations from the FHN to initiate projects under a new FQHC Incubator Project.
Since then, the FQHC Incubator Project has continued to focus on increasing access to health care among different target populations. To be eligible for funding, applicants must be an existing FQHC or FQHC look-alike within the State of Nevada.
SFY24-25 Grant Recipients Community Health Alliance Supports the development of a Medication-Assisted Treatment (MAT) program and integration with the existing Behavioral Health program for patients with Substance Use Disorder (SUD). Hope Christian Health Center Expands Community Health Worker program focused on identifying and analyzing social determinants of health and healthcare gaps experienced by the patient population.
Relationships with local resource organizations will be established, and patients will be connected to services to meet the needs not previously being met. Expands the mobile service line of Nevada Health Center’s Medical Van based in the Reno and Elko areas, serving geographically isolated patients in medically underserved rural and frontier communities.
Supports the hiring, training and development of staff in preparation of opening an additional FQHC location in Washoe County to provide primary medical care, behavioral health care, case management, and other services. Southern Nevada Health District Supports the implementation of an Integrated Behavioral Health Model into workflows and processes, and coordinates with other FQHC clinics to identify and refer Behavioral Health patients.
