FY 2025 Grants (Healthy Aging Guidelines) is sponsored by Samuels Foundation. Supports programs focusing on healthy aging, including free tax preparation for older adults, occupational therapy in supportive housing, health and wellness program expansion, care for older surgical patients, and case management for underserved older adults.

FY 2025 Grants | Samuels Foundation Free Tax Preparation for Older Adults in the Bronx This grant will expand a previously funded program that helps seniors file their taxes, obtain needed refunds and access other benefits and resources. $145,000 | Breaking Ground Implementing Occupational Therapy in Supportive Housing This grant will support an older adult Occupational Therapy program in supportive housing.

$100,000 | Bay Ridge Center Health & Wellness Program Expansion This grant will build on the success of the pilot health and wellness program to expand and enhance services aimed at improving member and community health.

$200,000 | Cornell University - Weill Medical College The Program for the Care and Study of the Older Surgical Patient The program aims to reduce the incidence of postoperative complications through comprehensive preoperative assessments, tailored interventions, and multidisciplinary care.

$120,000 | Hamilton-Madison House, Inc. Case Manager for Older Adults Not Served by the Existing Case Management Ecosystem This grant will fund a new case manager position, in order to serve older adults who do not fit criteria for existing programs. Adult Day Health Center (ADHC) Expansion This grant will support outreach to hard-to-reach older adults to increase enrollment in the specialized ADHC.

$200,000 | Icahn School of Medicine at Mount Sinai Age-Friendly Care for a Healthier Tomorrow: Expanding Geriatrics at Mount Sinai Queens This project aims to increase staff trained in the 4Ms framework and aligning care with each geriatric patient's needs and preferences.

$200,000 | Icahn School of Medicine at Mount Sinai Care Navigators at the Deane Center: Transforming Dementia Care Delivery This grant will support care navigators via the GUIDE model to provide a comprehensive care coordination, care management, caregiver education and support, and access to respite services.

3D (Desi Dementia Day) Care This program will provide culturally competent care, nutrition, caregiving assistance, and support service for South Asian older adults with mild to moderate dementia and their caregivers.

$183,000 | Jewish Association for Services for the Aged (JASA) Age-Friendly Medication Management for CHF Patients: A JASA–Maimonides Hospital-to-Home Partnership This program addresses persistent gaps in transitional medication management for older adults with congestive heart failure (CHF), aiming to reduce preventable readmissions and improve adherence, safety, and continuity of care.

$120,000 | Korean Community Services of Metropolitan New York, Inc. Expansion of KCS Manhattan Older Adult Center This grant will support the expansion of a new Older Adult Center. $250,000 | Maimonides Medical Center Age-Friendly Health System - AIM 2.

0 – Comprehensive Age-Friendly Care for Older Adults This grant will be used to recruit and hire an Age-Friendly Program Manager (AFPM) who will work to strengthen and expand Age-Friendly care.

$160,000 | Marks JCH of Bensonhurst Healthy Living for Older Adults This grant will support a geriatric social worker to lead and supervise an initiative that will train staff and enhance agency-wide collaboration to provide services to older adults.

$184,000 | Medicare Rights Center Helping New York City's Low-Income Older Adults Keep Their Medicare Cost Assistance Benefits This grant will help launch a new initiative to proactively reach and help low-income NYC older adults renew their Medicare Savings Program benefits.

$175,000 | Northwell Health Foundation Palliative Care Supportive Oncology Program at Staten Island University Hospital The program aims to address the significant gap in outpatient palliative care for older adults with advanced cancer on Staten Island.

$90,000 | Rutgers the State University of New Jersey Expanding Peer Support Services for Older Adults with Vision Loss in New York City This grant will fund the expansion of a program that provides structured peer support to older adults adjusting to later in life vision loss.

$200,000 | Supportive Housing Network of New York Older Adult Supportive Housing Advocacy Initiative This project will provide insight into the number of older adults in supportive housing and analyze where there are levers to enhance funding for capital improvements and enhanced services.

$75,000 | Volunteers of Legal Service, Inc. VOLS Senior Law Project/Legal Services for Chinese American Older Adults This grant will support the salary of a full-time bilingual staff attorney to increase legal services outreach and support to low-income Chinese American older adults in New York City.

$200,000 | YWCA of Brooklyn Independent Living and Wellness Support for Formerly Homeless Aging Residents This program will provide on-site independent living and wellness services to support their older formerly homeless residents.
