dist_schl=53_0101 May applicants provide services using Crystal Lake Elementary School space during afterschool hours? Yes. With permission from the Principal and/or Director of the Community Partnership School, space for afterschool activities can be scheduled.

For the Crystal Lake grant opportunity the RFP states that we need to submit 10 hard copies. Is this just 10 copies of the 3 page application or would you like us to include 10 copies of all the attachments?

10 copies of the RFP with one copy of the attachments should be fine For the community school, is the nonprofit required to “advertise” and recruit for the services provided or will the school identify those with the need and refer them? The applicant will need to recruit clients and should identify ways in which they hope to access individuals/families who will utilize their services.

Recruitment can include communication with the Community Partnership School Director, Afterschool Coordinator, Health Care Programs Coordinator and other non-profit organizations providing services in the area. Will students be allowed to receive services during school hours? Yes.

With permission from the Principal, services (speech therapy, tutoring, counseling, etc.) can be scheduled during school hours. Partners will be asked to schedule sessions in the afternoons as morning time slots are committed to required academic lessons and assessments.

Basic Crystal Lake information: Crystal Lake Elementary School is a Pre-K through 5th grade facility located in east Lakeland within the Central Florida School district of Polk County. The school campus is adjacent to Crystal Lake Middle School. Eighty-seven percent of exiting 5th graders attend Crystal Lake Middle.

Crystal Lake Elementary School is a Title 1 school where 100% of the students receive free or reduced lunch. They serve a diverse population consisting of 30. 3% White, 36.

2% Black, 28. 2% Hispanic,0 . 6% Asian, 0.

6% Indian, . 8% Hawaiian or Pacific Islander and 4. 5% Multiracial.
