Snyder – Treasurer Richard B. Patterson, DPM, MSPH Howard Nochumson – Executive Director *The Board of Directors mourns the untimely death (February 27, 2010) of Board Member, Sunny Wirtz. Sunny was a founding member of the Board of Directors and, until her recent illness, served as Vice President of the Board.
Sunny was known for her tireless advocating for the availability of health care services for the medically indigent. She also was one of the strongest proponents of cutting edge medical research, as exemplified in her support of the Foundation’s funding for the Chicago Diabetes Project.
Mission Statement Realizing the healthcare crisis in our country, the Board of Washington Square Health Foundation, Inc. recognizes that no one foundation can meet all the challenges of the healthcare environment.
However, the Foundation has developed a program of grant making which is designed to be both a catalyst and guide for other foundations and grant making organizations in meeting the various needs of the Chicagoland healthcare community.

Therefore, solicitation of individual members of the Washington Square Health Foundation, Inc. Board of Directors by potential recipients for specific Washington Square Health Foundation, Inc. grant requests is discouraged. Priorities for funding will vary from time to time. Even though a project meets established criteria, it may not be adopted because of limited funds or other priorities.
Washington Square Health Foundation, Inc. Grants do not merely provide funds to an existing project, however worthy. They must be designed to provide significant health related benefits. Projects must have the support of appropriate organizations and/or governmental units and in the community where the project is to be carried out.
To be eligible for consideration, projects must be efforts involving a not for profit tax exempt 501(C3) organization or association as sponsor. The grant request must be submitted by an organization exempt from income tax under sec. 501 (c)(3), and designated as a public charity under section 509(a)(1) or 509(a)(2), of the Internal Revenue Code.
Washington Square Health Foundation does not provide grants to… * Reporting The Washington Square Health Foundation requires progress reports on the use of its funds. If your grant request has been approved please be aware that Washington Square Health Foundation actively tracks these requests to assure that funds are being used in the most effective way, as identified in the grant agreement.
Six-month reporting periods require completed report forms. Unless otherwise indicated, the first six-month period begins when funds are received (actual payment date), and allows an additional month to prepare the report (i.e. funds are received 9/30/05, report will cover 10/01/05 – 3/31/06, and is due 5/01/06). The final report for one-year grant terms will cover the next six-month period (4/01/06 – 9/30/06, due 11/01/06).
The same form is used for all reporting periods (click here). In addition, at the time of the interim and final reports, the Washington Square Health Foundation requests that grantees provide professional quality photographs in digital format.
Please obtain and provide the Foundation with releases of liability and permission to utilize (for publicity purposes) statements from all individuals and/or their legal guardians included in or associated with any audiovisual documentation, photographs or slides provided to the Foundation.
