Grant Application - Healthy Houston Foundation A Legacy of Hope and Health The Healthy Houston Foundation, Healthy Texas, Healthy America dba Healthy Houston is a 501(c)(3) charitable foundation working to improve access to health care for underserved people. Our primary goal is to support the creation of Federally Qualified Health Centers.
Federally Qualified Health Centers are nonprofit, community-based clinics that offer preventative, primary care, dental and mental health, laboratory and x-ray, pharmacy, and prenatal and women’s health care services to low-income patients on a sliding scale basis. Specifically, Healthy Houston offers grants and loans to non-profit clinics, either existing or under development, that offer health care to medically underserved patients.
Grants do not need to be repaid while loans must be repaid at low interest to allow for other clinics to use resources at a later date. The Foundation may elect to contribute its funds to another foundation of like purpose. Download the Grant Application Form Here Grant Application Form Download
