Greater Rochester Community Health Foundation - Small Grants Program - NH Charitable Foundation The Greater Rochester Community Health Foundation (“GRCHF”) is a supporting organization of the New Hampshire Charitable Foundation.
It was established with the proceeds from the sale of the nonprofit Frisbie Memorial Hospital to HCA Healthcare, Inc. Its mission is to improve health and well-being and reduce the burden of illness of people in Strafford County and surrounding communities. Learn more about GRCHF and its vision and values . The focus of this grant program is to provide general operating support related to access to care.
This program is open to nonprofit organizations that serve Strafford County residents and work on access to care challenges. Access to care may include: substance use disorder services, food security, housing, senior services, transportation and logistics, culturally appropriate care, awareness of services, care coordination and patient motivation.
Priority will be given to collaboration projects along with applicants targeting maternal health. If your organization is seeking project-specific support related to mental health/behavioral health/substance use disorder services; early education/childcare or food security, please apply to the targeted grants program . Funding is limited to organizations providing services in Strafford County, New Hampshire.
If your organization is headquartered outside of Strafford County but considers Strafford County to be a primary service location, you are eligible to apply. Organizations must be recognized as tax-exempt under Section 501(c)(3) of the Internal Revenue Code. Organizations that do not have public charity status may apply for grants if another public charity acts as a fiscal sponsor .
