Health Care for Homeless Veterans (HCHV) Program

Health Care for Homeless Veterans (HCHV) Program is a grant from the U.S. Department of Veterans Affairs that funds community-based organizations providing health and supportive services to homeless veterans. The program contracts with community mental health centers, nonprofits, and other service providers to offer outreach, case management, and residential care to veterans experiencing homelessness across the country.

HCHV serves as a cornerstone of VA's broader effort to end veteran homelessness by connecting veterans with healthcare, mental health treatment, substance abuse services, and stable housing. Eligible applicants are community mental health centers, nonprofit organizations, and other service providers capable of contracting with the VA to deliver services to homeless veterans.
